Sunrise Country Manor seeks a Full Time Registered Nurse to act as the Assistant Director of Nursing (ADON) & MDS Coordinator for our 80 bed facility.

The ADON will serve directly under the Director of Nursing (DON) and will serve as leadership support over the nursing department. In the absence of the DON, the ADON will have administrative authority, responsibility and accountability for the functions, activities, orientation and in-service education of the nursing staff.

The MDS Coordinator will have administrative authority, responsibility and accountability for the functions of the MDS process. The MDS Coordinator will be responsible for gathering information from residents and their families through interviews using the Resident Assessment Instrument (RAI). This information will be used to formulate individualized person-centered care plans in coordination with social services, dietary, rehab specialists, and medical staff. MDS coordinators then implement and monitor these care plans to ensure their accuracy and effectiveness while complying with State and Federal regulations.

The ADON/MDS Coordinator will also be responsible for acting as a positive role model and leader in support of the mission, vision and values of Sunrise Country Manor.

Responsibilities include but are not limited to:

  • Assist the DON to govern the day-to-day functions of the nursing service department.
  • Assists the DON by participating in the facility’s personnel process to include recruitment, hiring, education, termination, disciplinary hearings and grievance hearings when necessary.
  • Lead and coordinate the MDS process including monitoring input and timeliness, organizing information and completing care plans
  • Develop, manage and coordinate the calendar of scheduled assessments in compliance with State and Federal regulations
  • Ensure care plans accurately reflect the current condition of every resident and that Medicare RUG levels are met
  • Work closely with an multi-disciplinary care team to coordinate cohesive care and documentation

Required Skills:

  • Must be a graduate of an accredited school of nursing with a current, unencumbered license to practice as a Registered Nurse (RN) in the State of Nebraska.
  • Must have a minimum of 2 years equivalent to full-time experience in long term or acute care setting.
  • Must have a basic understanding of the MDS process and the RAI rules and regulations.
  • Must have nursing assessment skills and the ability to conceive and create care plans based on comprehensive resident assessment that is accomplished through resident and family interviews, physical assessment, observation and input from a multi-disciplinary care team.
  • Prefer previous experience with PCC (Point Click Care)
  • Strong organizational skills
