Location: Dallas, TX
Pay: $16,502 per year stipend (required # of hours: 1,700/year)

Day-To-Day
- Develops trusting relationships with clients by providing support and advocacy to help achieve health goals
- Conveys the purpose and services of Community Health to the member population and the impact that program or service.
- Meets member in clinic, facility, office, or at home to help identify social determinants of health impacting member's health and general well-being
- Follow-up with health management/care plans with both members and providers
- Coach member in effective management of their chronic health conditions and self-care.
Qualifications
High School diploma or equivalent
Any combination of 1 years health/social services experience and/or education
- Prefer candidates who live within a 10 mile radius of zip codes 75208 & 75211
- Valid Texas Driver s License and ability to be insured.
