Supportive Living Assistant:
*** Must be EXPERIENCED with APD and SLC responsibilities***
-Proof of training required
-Demonstration of knowledge required
This position is to provide administrative assistance to the Supportive Living Coaches within the agency.
Office Hours are REQUIRED----- 2-3 {6-Hour days in Agency Office}
REQUIRED DUTIES:
-Maintain Calendar for all (SLC) Supportive Living Coaches cases
-Creating and noting Appointments for SLC clients by creating calendars for coaches outlining all appointments for all SLC clients. Providing proof of correspondence to HR Manager via email
-Noting Quarterly Meeting Updates on SLC calendars and in i-connect documentation. Providing Calendars to HR Manager via email
-Noting Support Plan meetings on SLC calendars and in i-connect documentation.
***SLC Liason is responsible for providing written correspondence when possible of all communications on a client’s behalf via email to Agency office email.
SCL i-connect and SLC documenting
-Audit Dates/Time entries in i-connect for all SLC personnel and include details on task sheet
-Log SLC progress notes for all SCL clients each week.
-Upload all signed documentation pertaining to client financials, medical, and housing information
-Maintain SLC Client books kept in Office by keeping documentation in the designated binders up to date
**SLC Liason is responsible for all sensitive information in each binder.
SLC APD duties
-Upload all documentation requested from WSCs or Guardians pertaining to SLC clients :
-Quarterlies, Implementation Plans, All Summaries, Annuals and all other SLC documentation required by APD.
Level 2 Background Check Required (LiveScan) required to work for Maintain Calendar for all SLC cases
