Job Title: Be@School Coordinator
Summary:
Be@School Coordinator will support anti-oppressive/anti-racist practice, and trauma-responsive practice in all aspects of their interactions with staff, community and company stakeholders through their administrative and operations support of program staff working on site and in community to address socioeconomic and health disparity. Provide parent mentoring services as outlined in the client's parenting plan in the home to help children and adults live successfully and avoid systems of oppression. Provide short-term case management services to families experiencing barriers to consistent school attendance.
Essential Duties and Responsibilities:
- Maintain client caseload service expectations to ensure client care is achieved: 25-30 cases
- Provide intensive outreach to engage families experiencing attendance concerns and collaborate for service delivery
- Provide short-term case management to families experiencing barriers to attendance at school and make appropriate community connections
- Connect families to community resources and advocate for them within oppressive systems
- Encourage service retention and regular communication, coordinate services with other health care providers to ensure that client receives adequate services
- Build relationships and connections with families to ensure quality care is the foundation of the supports
- Provide adequate documentation of services in a timely fashion
- Participate in consistent supervision- for professional development
- Engage community providers for successful partnerships and collaboration
- Ensure compliance with county and grant contracts
Qualifications Education/Experience:
- Bachelor's Degree in family social services, social work, behavioral health/psychology, or another related degree
- 2-3 years working with individuals and families working on addressing behavioral health needs and goals
- Strong written communication and comfort communicating by email to share updates and questions with key stakeholders, edits work for spelling and grammar and able to vary writing style to meet needs
- Strong oral communication demonstrated through the ability to speak clearly and persuasively any situations, understands how to actively listen and seek clarification and responding well to questions.
- Bilingual in Spanish preferred
