Shift: 9AM-7:30PM (4 10 hour shifts with rotating weekends)

ESSENTIAL JOB FUNCTIONS

  • Completes the Admission/Discharge Log and paperwork associated with the intake/admission process and releases of information. Reviews ISPs, closes appropriate program codes in the EHR, administers the Consumer Satisfaction Survey, and completes the transition/continuity of care plan.
  • Participates in the coordination of client discharges with the interdisciplinary team.
  • Checks food prep, inventory, and other Monitor duties
  • Provides training on living and social skills to clients. Reviews and reinforces safety plans and skills learned with clients
  • Transports clients to outside appointments as needed
  • Assists clients with self-administration of medications as necessary. Coordinates medication pick up with the unit nurses as needed.
  • Observes, documents, and reports client changes in emotions, mental status, and behavior
  • Collaborates as appropriate and coordinates care with social service providers, medical care providers, family members, landlords, and other Nord Center staff associated with consumer's treatment plan. Implement ISP's to support the optimal functioning in the least restrictive environment with consumers’ participation in additional settings, services, and levels of care.
  • Triage telephone calls. Gather the information needed to determine if an emergency assessment or other emergency assistance is required. Provide information and referral to persons with more routine requests. Provide telephone support and crisis intervention to clients who call the hotline.
  • Provide/Coordinate services including, but not limited to, skills training recovery, advocacy and linkage to supportive services to meet medical, entitlements meaningful activity and entitlement needs. Ensure that follow up care is provided in an appropriate and timely manner according to medical necessity.
  • Completes outcomes within the time-guidelines of their due dates (initial, 6 months and annual)
  • Completes progress notes in a timely manner. Clinical documentation is completed within the 24 hour guideline and reflects services rendered as well as medical necessity
  • Maintain required productivity expectation.
  • Attends required staff meetings.

  • Maintains proper/professional boundaries with others.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
In order to perform the functions of this job, the employee must possess the following: a working knowledge of Nord Center Policy and Procedures, including sensitivity and adherence to clients’ rights, confidentiality, and health and safety issues. Must exhibit sensitivity to different cultures.

WORKING CONDITIONS
The work is provided mainly in the office and on the care unit, but may require travel to outside locations. Because of the variety of environments and physical conditions that will be encountered, this individual must be able to successfully interact and navigate in all types of facilities and weather conditions. This includes being able to walk, climb stairs, navigate changing surface conditions including wet, slippery, and icy grounds, and function in the proximity of electrical appliances, open flames and in areas with poor air quality including potentially smoke-filled rooms. This individual may periodically be required to lift and/or carry up to 10 pounds. Must be able to physically interact with client and others in emergency situations to prevent harm to client and others. The individual in this position must be able to communicate clearly both verbally and in writing. Due to the extensive computer work this individual must be able to sit for extended periods of time and operate required equipment.

REQUIREMENTS/QUALIFICATIONS
Bachelor's Degree in psychology, social work, sociology or related field and prior experience required; LSW, CT or SWT is preferred. Experience in the field of mental health is preferred. Basic computer skills required, including familiarity with Windows and Microsoft Office. The necessity for travel in this job requires that the employee possess a valid Ohio driver's license, a clean driving record, have proof of adequate personal automobile insurance, and use of a personal vehicle. Knowledge of Dialectical Behavior Therapy is a plus.

Bilingual (English/Spanish speaking) a plus.

Experience with EHR (Electronic Health Records) a plus.
