Description:POSITION SUMMARY:
The Care Navigator is responsible for providing care coordination, resource linkages, advocacy, and emotional support directly to isolated LGBTQ+ seniors and adults with disabilities. This person plays a critical role as the liaison between our community members, Case Management team, Housing and Resources Team, Community Engagement Team, and outside agencies. The Care Navigator acts as an Openhouse triage, intersecting and working closely with community members and the Openhouse programs staff to ensure they are connected with the resources they need to age in place and help navigate the systems that provide the care they need. The position is full-time with full benefits and reports to the Manager of Community Support Services. This person works closely with the Manager of Community Support Services to provide reliable, compassionate, and consistent personalized service connection for LGBTQ+ seniors and adults with disabilities. This position is on site at our Openhouse office at 65 Laguna and requires regular in-person visits to clients’ homes in order to meet the needs of our community members.
ABOUT OPENHOUSE:
Openhouse was founded in 1998 to enable lesbian, gay, bisexual, transgender, and queer (LGBTQ) seniors to overcome the unique challenges they face as they age by providing LGBTQ-welcoming housing, resources, high-impact direct services, and innovative community programs. As a result, Openhouse reduces isolation and empowers LGBTQ seniors to improve their overall health, well-being, and economic security.

COMPENSATION:
Starting range is $23.50/hour to $25.75/hour. Compensation is commensurate with experience. This is a full-time (1.0 FTE) non-exempt position. Openhouse offers a dynamic, vibrant, collaborative and supportive work environment. All eligible employees may participate in insurance benefits upon first of the month following 30 days after date of hire – full-time employees are eligible for health insurance, while full- and part-time employees are eligible for dental, vision, and LTD insurance. 1. Intake and Ongoing Assessments

  • Complete intake assessments for each client. Develop an individual, person-centered and strengths-based plan to address the service needs identified in the assessment. Set clear expectations about the parameters of the Care Navigator’s role and about both intake and discharge processes.
  • Assess community member’s functional status, strengths, and limitations, including identification of issues and challenges in their home environment, social support network (family and friends), caregiving responsibilities, housing status, financial security, food security, transportation, access to primary and specialized health care. Additionally, assess level of community engagement and participation in health and wellness activities. Lastly, assess client’s interests and challenges in seeking support. This is a non-clinical assessment through asking questions to community member and caregivers, if applicable.
  • Ensure the implementation of the client-approved care navigation plan by coordinating and arranging services as necessary based on the client’s functional capacity as determined in the assessment.
  • Consistently evaluate the quality and effectiveness of the services provided to the client and client’s satisfaction with services.
  • Provide crisis support, care coordination, and individualized service navigation.
  • Assess clients’ needs and eligibility for volunteer peer support and work with staff to secure appropriate volunteer matches.
  • Conduct regular client check-ins to provide on-going emotional and practical support, identify shifts in baseline needs and the necessity for new referrals, and assess satisfaction with services and, if appropriate, peer support.
  • Field calls from LGBTQ+ seniors and adults with disabilities, their caregivers and providers about the program.

2. Assistance and Follow Up

  • Advocate on client’s behalf, as needed with housing, health, and social service providers.
  • Refer clients to case management, home care, mental health, transportation, legal, estate planning, memory care, and other services as needed.
  • Follow up with clients and community partners to ensure receipt of services and maximize client utilization of services and programs; document the outcome of referrals and client satisfaction.
  • As needed, assist community members with completing the application process for Club 75 – the community day program that Openhouse co-manages in partnership with On Lok; and PACE – the Program of All-Inclusive Care for the Elderly run by On Lok.

3. Friendly Visitor Program Collaboration

  • Work with the Community Engagement Coordinator (CEC) to support Friendly Visitor volunteers when community members experience changes in health requiring higher level of care and support.
  • Regularly discuss with CEC ways to best support community members and volunteers enrolled in the Friendly Visitor program when challenges arise with senior matches.

4. Reporting Data Collection and Oversight

  • Collect and report data on a monthly basis for funding sources regarding units of service units delivered.
  • Complete and maintain intake forms for each client and volunteer, including basic demographic information, income-level and sources, health status, housing and service needs and other appropriate information.
  • Administer Openhouse program policies, procedures, information collection, reporting and evaluations.

5. Discharge

  • During intake and throughout clients’ enrollment in Care Navigation, communicate criteria/protocol for discharge and reinforce that the Care Navigator is involved with clients for a limited period of time.
  • Follow clear and consistent protocol for all client discharges.
  • Document discharge process, including achievements, goals met, and referrals made throughout client’s enrollment in Care Navigation.

DESIRED QUALIFICATIONS:
The ideal candidate for the position will be able to demonstrate:

  • Explicit desire and passion to work with LGBTQ+ seniors.
  • Experience with and understanding of the unique housing, medical and psychosocial challenges of lesbian, gay, bisexual, and transgender seniors and adults with disabilities.
  • Exceptional written and verbal communication skills, ability to professionally connect with LGBTQ seniors, adults with disabilities, their caregivers and support systems and teams of health professionals.
  • Demonstrated experience providing information and assistance, care coordination, and linkages to the community health care delivery system.
  • Understanding of age-related cognitive decline.
  • A social justice-oriented mind set.
  • Experience in motivational interviewing, counseling, advocacy, de-escalation, and/or behavioral health.
  • Successful experience working in culturally diverse situations including working with people of various racial and ethnic backgrounds, sexual orientations and gender identities.
  • Proficiency in multiple languages.
  • Experience working with older adults or equivalent experience working with other populations with unique socialization and service needs including homeless communities and veterans.
  • Sensitivity to and knowledge of the unique life experiences, current challenges, and strengths of LGBTQ community members.
  • Proficient with Microsoft Office Suite and the ability to quickly learn software programs and online databases and tools.
  • Good problem-solving skills and the ability to prioritize multiple tasks.
  • Excellent time management skills.
  • Attention to detail and ability to provide high quality data and reports.

EDUCATION/EXPERIENCE:
Bachelor’s degree in health, social work, psychology, counseling, gerontology, other related fields plus one year working with seniors and/or adults with disabilities. OR three years equivalent experience.
PHYSICAL REQUIREMENTS:
Physical skills and ability to perform work that requires sitting, walking, stopping, bending and lifting up to 35 pounds. This position is on site at our Openhouse office at 65 Laguna and requires regular in-person visits to clients’ homes in order to meet the needs of our community members.
