Job description

This Case Manager will provide case management services to youth and families.

Responsibilities:

• Interviews youth and families and prepares case management needs assessments, evaluates data gathered in consultation with Clinician, develops appropriate Needs and Services Plan which will provide maximum benefit for youth and families, refers to appropriate services and community agencies as necessary.
• Facilitates Child and Family Team Meetings and coordinates the participation of team members.
• Collects and revises information on community health and welfare resources. Acts as an advocate in the coordination of community services. Refers youth and families to appropriate agencies as needed.
• Works closely with clinical staff and collaborates with members of the Child and Family team.
• Communicates observations, facts and comments to supervisor. Documents in Exym as required and maintains required records, reports and statistics.
• Plans, organizes and maintains case management services and facilitates program development.
• Collaborates with community agencies, schools, the Child and Family team to meet youth/family needs.

Requirement:

• Bachelor’s degree in a Behavioral Science from an accredited college or university and a minimum of two (2) years full-time experience in a public or private social services setting working with children, youth and transition age youth.
• Bilingual English/Spanish preferred.
• Obtain, within 30 days of employment, Pro-ACT, Red Cross CPR and First Aid Certification and maintain such certification current through the term of employment with SGCC.
• Maintain a valid driver’s license and provide proof of insurance if transporting clients/families. Must have held a valid driver’s license for a minimum of 5 years.
• Must be able to obtain and maintain Live Scan clearance, pre-employment physical, TB test and drug test.

Pay Scale - $22 an hour
