POSITION SUMMARY

Instead of a “one size fits all” approach, our expert staff of addiction specialists works directly with patients to effectively craft a personalized treatment program to treat alcohol and substance. Our organization is looking for dynamic individuals who love to learn, thrive on innovation, and are open to exploring new ways to achieve our goals.

1. Provide appropriate contact, resource and referral information for all clients to include, but not limited to, information related to discharge planning, emergency placements, medical issues, domestic violence, pregnancy, CYS, legal issues and transportation.

2. Initiate contact with families providing education on visitation policies and promote support/involvement

3. Assist with coordination of external supports, to include but not limited to, external case managers, Certified Recovery Specialists, peer specialists, and sponsors

4. Assist with insurance/funding issues, to include but not limited to, benefit termination, county funding and disability eligibility.

5. Assist with medication issues as necessary

6. Provide counseling support services, as necessary

7. Participate in Treatment Team Meetings

8. Create, revise and conduct educational sessions, as assigned

9. Prepare and maintain required documentation, records and reports in compliance with all hospital regulations.

10. Complete all relevant paperwork and correspondence needs relevant to patient’s served

11. Attend hospital and staff meetings, educational programs and in-services as required.

12. Represent the Social Services department in hospital generated committee meetings as assigned.

Education: Bachelor’s degree in Social Work, Psychology, or another related field. Preference for a minimum two years’ experience in a social work setting, with mental health or addictions emphasis, providing direct service to clients. One year experience required!

Job Type: Full-time

Pay: $48,000.00 - $55,000.00 per year
