Case Manager II - Diversion

East Brunswick, NJ

Salary: $43,000

Responsibilities

• Provide Middlesex County households, who are at imminent risk/currently homeless, with strength-based approach services and financial support in order to assist with retaining or obtaining a safe stable living situation and avoid/exit homelessness
• The job includes screening, assessment, community outreach across Middlesex County, service planning, crisis intervention, advocacy, housing assistance, linkage, life skills teaching and utilization of HMIS case management software
• Connect Homeless Clients to needed services to breakdown barriers to remain housing.
• Provide case management services to individuals, assess, service plan, linkage, crisis intervention, de-escalation, manage relationship with a broad range of community providers.
• Provide documented case management planning and work on client goals. Document progress and assist in obtaining any personal items to fulfill client needs.
• Life skills teaching and housing retention.
• Assist clients in applying for housing, provide transportation for individuals to housing opportunities.

Requirements

• Evening hours, driver’s license with acceptable driving history, BA/BS (requirement)
• Bi-lingual encouraged to apply. Ability to work independently
