Job description

We are seeking a case manager in our Warren, Ohio office to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder.

The Case Manager will assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have a high school diploma with three years’ experience but prefer a BA in related field, one year related experience and knowledge of community resources. Must have and maintain a clean driving record.

Must have an NPI number and Medicaid Provider Number or be willing to obtain such.
