Summary: Provides and coordinates care for hospice inpatients.

Duties/Responsibilities:

• Provides direct nursing care appropriate for a licensed RN.
• 6+ months of experience in a Charge/Leadership role.
• Participates in the development of the patient’s plan of care unique to the individual patient/family needs and coordinates the services/activities of the multidisciplinary team in respect to the plan of care.
• Documents activities and assessments in the clinical information system and appropriately updates team members.
• Provides support to patient’s family/friends including education, expectations, and reporting changes in condition.
• Coordinate admissions, transfers, and discharges.
• Make staffing assignments for RN, LVN, CNA.
• Guide employees to provide the highest level of care using best practices, and to do it with confidence and compassion.
• Participate in the identification of staff educational needs.
• Work directly with Interdisciplinary Team members, patients, and visitors to ensure excellent customer service under the supervision of Patient Care Manager.
• Lead by example and support the organization’s goals, initiatives, and value system.
• Understands and is compliant to organizational policies and processes relating to regulatory, accreditation and privacy standards.

Qualifications:

• Graduate of an accredited school of nursing.
• Current Texas license.
• Current certification in basic life support.
• Ability to function as part of an interdisciplinary team.
• Good interpersonal skills – able to effectively relate and communicate with team members and patients from a diverse community.
• Knowledge and experience in a hospice or hospital setting is preferred.
• Experience using a clinical information system is preferred.
