Job description
Summary: Provides and coordinates care for hospice inpatients.
Duties/Responsibilities:
- Provides direct nursing care appropriate for a licensed RN.
- 6+ months of experience in a Charge/Leadership role.
- Participates in the development of the patient’s plan of care unique to the individual patient/family needs and coordinates the services/activities of the multidisciplinary team in respect to the plan of care.
- Documents activities and assessments in the clinical information system and appropriately updates team members.
- Provides support to patient’s family/friends including education, expectations, and reporting changes in condition.
- Coordinate admissions, transfers, and discharges.
- Make staffing assignments for RN, LVN, CNA.
- Guide employees to provide the highest level of care using best practices, and to do it with confidence and compassion.
- Participate in the identification of staff educational needs.
- Work directly with Interdisciplinary Team members, patients, and visitors to ensure excellent customer service under the supervision of Patient Care Manager.
- Lead by example and support the organization’s goals, initiatives, and value system.
- Understands and is compliant to organizational policies and processes relating to regulatory, accreditation and privacy standards.
Qualifications:
- Graduate of an accredited school of nursing.
- Current Texas license.
- Current certification in basic life support.
- Ability to function as part of an interdisciplinary team.
- Good interpersonal skills – able to effectively relate and communicate with team members and patients from a diverse community.
- Knowledge and experience in a hospice or hospital setting is preferred.
- Experience using a clinical information system is preferred.
