POSITION OVERVIEW:

The Client Care Coordinator is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/caregiver introductions and quality assurance visits with existing clients. They determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction, and retention. This position works closely with caregivers to offer support, feedback, coaching, and training.

This position will support the Overland Park, KS and Kansas City, MO Offices.

RESPONSIBILITIES:

This position serves as a first touch point for new clients, and as an account manager for existing clients. In addition, the position plays a key role in identifying client needs and matching appropriate services.

  • New Client Intake Assessments - Contact new clients to schedule and perform remote or in-home assessments for start of care.
  • Create new client profile and care plans through the entry of accurate client data into FirstLight Home Care software systems
  • Assist scheduler with scheduling of shifts by matching caregiver qualifications and availability to clients’ needs
  • Conduct client/caregiver introductions with every new client and with every new caregivers
  • Case Management - Perform ongoing remote and in-home evaluations, caregiver introductions and scheduled supervision of services to ensure standards of care are met.
  • Supervise and coach caregivers and assist in conducting performance appraisals
  • Accurately identify and report changes in client condition to agency employees involved in the client’s care plan
  • Participate in the quality assurance reviews and evaluations of the agency’s services
  • Assist with after hours on-call scheduling coordination periodically throughout the year.
  • Express proficiency in all FirstLight Home Care services, policies, benefits, and market differentiators.
  • Work toward continuous quality improvement and provide unparalleled customer service for all FirstLight Home Care clients.

QUALIFICATIONS:

  • Experience in home care, health care, or senior-related industry required, an equivalent combination of education and work experience may be considered.
  • Bachelor's Degree or higher in a related field is highly preferred
  • Knowledgeable on assisting clients with personal care services and can train others.
  • Nursing Background or Social Work Background preferred
  • Proficient with computers and Microsoft Office
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Demonstrated experience in staff mentorship, training or supervision
  • Working knowledge of the home care/home health industry.
  • Self-motivated with an extremely strong sense of urgency.
  • Flexibility to adapt and change as priorities evolve in a rapidly growing environment.
