Job Title
Clinical Intake Coordinator
Summary
The Clinical Intake Coordinator strives to standardize the process of converting referral opportunities into admissions through efficient clinical and financial document review. The Clinical Intake Coordinator assists with patient transition to the facility by ensuring a thorough document review and verification of clinical needs have been completed.
Job Duties
- Prioritize incoming referrals from multiple hospital systems
- Initiate contact with referral sources or Transitional Care Coordinators to collect additional information
- Will collaborate with Chief Nursing Officer on clinically complex cases
- Ability to transcribe referral information into Point Click Care
- Review referrals for medications, durable medical equipment, additional clinical needs
- Responsible for End of Day Summaries and daily updates to external teams
- Give timely responses on accepting or denying incoming referrals
- Facilitate communication will multiple skilled nursing facilities on incoming admissions
- All other duties as assigned as this is not a comprehensive listing
Requirements
- Must hold a State of Illinois Registered Nurse License or above
- Must have Skilled Nursing/ Long Term Care experience and previous Director of Nursing experience
- Must have strong skills reviewing medical charts electronically
- Knowledge of Skilled Nursing/ Long Term Care clinical capabilities
- Ability to navigate Microsoft Outlook, Microsoft Word, Microsoft Excel
- Ability to multitask and collaborate with different team members
- Knowledge of Point Click Care is preferred
