Transitions Healthcare North Huntingdon is currently hiring a professional relationship builder to serve as our Clinical Liason. This positions will serve to to facilitate a smooth clinical transition of residents from their current setting of hospital, home etc. to our facility.
Clinical Liason Job Highlights:
· Respond to inquiries or referrals from hospital discharge planners, families and other sources in person or by phone in a timely manner.
· Manage the transfer process by always maintaining updated bed availability and facility services and capabilities.
· Manage the inquiry process professionally, timely and with appropriate follow up.
· Provide education as necessary regarding facility services and capabilities to medical insurance, legal and financial professionals as well as senior organizations, appropriate special interest groups, hospital discharge planners and other community contacts.
· Develop and maintain relationships to advise referral sources of bed availability and new products and services, and efficiently meet the daily needs of the community at large.
· Gather all necessary clinical information for consideration for admission as well as all clinical information for facility nursing department admission process
· Obtain access and efficiently utilize the referral database software systems for nursing to bring over to the EMR necessary demographic information upon admission
· Conduct tours of the facility with potential residents and/or their responsible party.
· Visit all referral sources regarding referrals timely and frequently including hospital and assisted livings, etc.
· Maintain a strong working knowledge of managed care and insurance coverages to appropriately answer questions.
· Gather any necessary medical documents to assist with insurance authorizations
Clinical Liason Requirements:
· Licensed Practical Nurse in good standing
· Two years equivalent experience in hospital liaison/marketing or working in an Admissions Department in a healthcare setting.
· Computer skills to include experience with Microsoft Office, Point Click Care, and hospital discharge systems.
