JOB DESCRIPTION

Summary/Objective

This position enables patients to stay in their homes by providing health and personal services; supporting Home Care patients and their families by meeting client's personal care needs and promoting client's comfort and independence in the home setting.

Essential Functions

  • Assists RN and/or LPN in direct client care including, but not limited to, providing basic personal care and hygiene assistance to clients, following the assigned clients' individual Master Plans, (i.e. bathing, oral care, dressing and grooming), within scope of practice.
  • Understand and strictly follow all applicable Home Care of the Grand Valley , Life Adjustment Program, Home Care, and State and Federal rules, regulations, policies, and procedures.
  • Complete required documentation promptly, accurately, and legibly, turned in to appropriate location timely.
  • Attend required staff meetings and case conferences 5. Provide behavior management as required (i.e. diffusing anger) 6. Contribute to the coordination of services through effective and timely communication with other members of the health-care team regarding any changes in client mental, physical, or environmental conditions or any client incidents.
  • Perform housekeeping duties including mopping, dusting, trash removal, linen change, stocking supplies, vacuuming, disinfecting, cleaning bathrooms, living rooms, wheel chairs and walkers, and organizing living space.
  • May assess patient condition by monitoring vital signs; recording intake and output; reviewing patient care journal; keeping family informed.
  • May administers treatments by dispensing medications; changing bandages; administering oxygen; maintaining oxygen supply and equipment
  • May help family members care for the patient by introducing and teaching appropriate ways to lift, turn, and reposition the patient; putting the family in contact with resources for additional home care help.
  • Update job knowledge and maintains certified nursing assistant status by participating in educational opportunities and fulfilling continuing education requirements.
  • Other duties as assigned.

Supervisory Responsibility

This position has no supervisory responsibilities.

Required Education and Experience

  • High school diploma or GED and a Certified Nursing Aid Certification (C.N.A.), and three months of related experience; or equivalent combination of education and experience.

Language Skills

  • Ability to read, write, and present information one-on-one and to small groups in English is required.

Work Environment: The following work environment characteristics represent those an employee typically encounters while performing the essential functions of this position.

This job operates 60% of the time indoors; 40% of the time outdoors. Occasional exposure to physically and/or verbally aggressive clients. Noise level is generally moderate. Job tasks involve exposure to blood, body fluids, and/or tissue.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee will stand 50% of the time, sit 10% of the time, and walk 40% of the time. The employee frequently is required to climb, balance, stoop, kneel, crouch, crawl, and reach with arms.

The employee must be able to grasp, pinch, and feel with hands and fingers and be able to lift, carry, push, pull, or otherwise move objects up to 50 lbs. frequently and up to

100 lbs. occasionally. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Specific hearing abilities required by this job include ability to perceive the nature of sounds at normal speaking levels with or without correction. Must be able to receive detailed information through oral communication, and to make the discriminations in sound.

Position Type/Expected Hours of Work

This is a full-time position typically Monday through Friday. Occasional evening and weekend work may be required as job duties demand.
