Posting Summary
Rutgers, The State University of New Jersey is seeking a Community Coordinator for the Department of Psychiatry within the New Jersey Medical School.

The primary purpose of the Community Coordinator, as a member of the Center of Excellence (COE) team, is to build relationships with individuals with opioid use disorder (OUD) in order to support the patient’s journey towards recovery. The Community Coordinator will provide continuity of care by meeting patients in various clinical care settings, including during emergency department visits, addiction treatment appointments, medical appointments, social service appointments, as well as offering individual and group counseling. They will also perform outreach by meeting with patients in the community. Individuals with personal experience with recovery from OUD or substance use disorders (SUD) are encouraged to apply.

Among the key duties of this position are the following:
  • Engage patient upon diagnosis of OUD in a clinical setting within the Rutgers University Hospital System (i.e.ED, medical clinic, social services office, etc.)
  • Assess patient wellness needs and strengths. Assists patient in developing recovery goals and formalized recovery plan.
  • Provide educational information and resources as needed.
  • Link patient to appropriate clinical and social services.
  • Accompany patient to referrals as needed.
  • Provide reminder calls for all upcoming appointments and accompany patient as needed.
  • Maintain all required documentation of encounters and recovery plans in EMR.
  • Identify and assist patients in overcoming barriers to recovery goals and participation in medical or mental health services.
  • Collaborate with physicians, social workers, nurses and other members of healthcare team serving patients with OUD in providing services.
  • Participate in clinical and programmatic level meetings and ongoing professional development trainings.

Qualifications

Minimum Education and Experience
  • Bachelor’s degree or equivalent, or a high school diploma plus equivalent years of experience.
  • Education and/or training may be substituted for the Bachelor’s degree requirements.
  • Applications from individuals with personal experience with recovery from OUD or SUD encouraged.
  • Must be able to work independently and as a member of a team.
  • Have good knowledge of public transit system or willingness to learn and teach.
  • Be able to use computer to record services or be willing to learn.
  • Must possess own vehicle or alternate means of transportation for use to outreach individuals served within Newark, NJ.


Certifications/Licenses
  • Must be certified or working toward eligibility for Certification as a CADC or related discipline.
  • Valid driver’s license.


Required Knowledge, Skills, and Abilities
  • Demonstration proficiency in Microsoft Office (Outlook, Word and Excel) is required.
  • Excellent customer service skills required.



Preferred Qualifications
  • Bilingual skills (English/Spanish).

Physical Demands and Work Environment
PHYSICAL DEMANDS:
Ability to speak clearly and write proficiently. Hearing is essential. The ability to focus
on the task at hand despite numerous interruptions. Must be able to sit for extended periods of time and document into an electronic medical record. The employee will spend time driving, walking, standing, bending and talking with individuals served in the community located throughout the County. May transport of individuals served in state vehicles. The employee can expect to lift up to 30 pounds.
WORK ENVIRONMENT:
The work environment includes both office based services, hospital-based settings and
community outreach. Individual wellness checks and crisis intervention required. Subject to unpredictable situations, patient crises, to include behavioral, medical and psychiatric crisis. Serves assigned shift which may include one (1) weekend day. On-call duties as assigned.
