Job Opening: Community Health Worker
Department: Delaware County Health Department, Population Health Division
Salary: Hourly: $42,003 per year, $21.54 per hour
Shift: 8:30 am to 4:30 pm, extended hours as needed
Delaware County (DelCo) is part of the new 5th congressional district and consists of over 184 square miles divided into 49 municipalities with a population of over 566,700 residents. Summary: The Community Health Worker will work under the direction of the Population Health Administrator to provide resources for county residents to improve individual and overall population health through the delivery of culturally sensitive, needs-based community outreach and education services. The Community Health Worker will have a working knowledge of the principles of Public Health 3.0. The Community Health Worker will act as a frontline ambassador of the DCHD establishing positive community partnerships while demonstrating and communicating the Health Departments mission, vision, and values.
Essential Duties:
· Emphasize collaborative engagement and resource sharing in order to positively address social determinants of health and improve health equity.
· Identify gaps in health and health-care related services and provide community-based outreach to promote public health.
· Effectively communicate with area schools, stakeholders, anchor institutions, and community-based organizations the role of DCHD and the various programs and services offered.
· Promote DCHD services and resources, including Wellness Centers and Immunization Clinics to increase awareness and service utilization.
· Complete and maintain accurate recordkeeping including client-focused documentation and organizational records.
· Track and document all contacts and interactions with clients, key partners, organizations and community representatives.
· Complete and submit bi-weekly productivity reports to direct supervisor and Pop Health Administrator.
· Distribute information and educate participants and community members on the importance of Primary Care Homes and Preventative Care to reduce the likelihood of chronic disease and illness.
· Aide residents in overcoming barriers to healthy living by connecting them to resources and ensuring access to social services and health related care including housing, food pantries, childcare, parenting classes, and mental health services.
· Establish positive, supportive relationships with participants and maintain positive working relationships with providers, agency representatives, supervisors, and office staff.
· Work to reduce cultural and socio-economic barriers between participants and institutions.
· Promote public health within the community.
· Attend all departmental meetings and assigned professional development trainings, and support DCHD committees and workgroups as directed.
· Safely operate county vehicles when necessary.
· Assist with loading and unloading supplies for outreach events.
· Assist preparedness team with emergency management activities as needed.
· Performs other duties as required.
Physical Requirements
Although most work performed is sedentary in nature, candidates must possess the ability to walk, stand, kneel, sit, squat, bend, and life up to 25 lbs. on occasion.
Qualifications:
· Valid driver’s license and ability to pass a background check
· High School degree required, Bachelor’s degree in public health, or another related field preferred
· Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
· Strong outreach and community engagement skills
· Knowledge of up-to-date community agencies and resources
· Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
· Knowledge of transportation and other barriers to care that may be encountered by client
· Demonstrated experience effectively leading and directing staff and/or volunteers
· Ability to maintain and handle confidential information
· Ability to follow protocol, procedures, and established guidelines
· A strong work ethic and ability to manage a demanding workload
· Ability to succeed under changing circumstances and a startup environment
· Strong relationship-building skills
*The selected candidate will be required to obtain, at their own expense, 3 forms of clearance, including PA State Police Criminal History Record Check; PA Child Abuse History; and FBI Criminal History Background Check including finger printing prior to employment.
