The Opportunity: The purpose of this position is to integrate Community Health Worker (CHW) into COVID-19 response to help mitigate the spread and effects of COVID-19 on underserved and underrepresented communities particularly racial/ethnic minority communities in Utah. This is an Alternative Funding Position.

Example of Duties:

• Create connections between vulnerable populations and the public health care system.
• Promote and support testing and vaccinations by mobilizing and educating communities.

• Identify people in need of services and support through local screening tools and testing follow-up.

• Connect community members and organizations to social services and resources.

• Use organizational outreach platforms to relay reliable information about COVID-19.
• Gather and report communities' experiences, perspectives, and attitudes regarding education and awareness.

Required Knowledge, Skills and Abilities: Proficient skill in the operation of personal computers and work processing software (Word, spreadsheets, databases). Ability to establish and maintain effective working relationships with employees, other agencies, and the public; ability to follow written and verbal instructions; ability to communicate effectively, verbally and in writing; ability to deal with the public under conditions of stress and maintain composure and professionalism. Ability to work independently on assigned projects. Must be able to lift 20 pounds and climb stairs. While performing the duties of this job the employee is frequently required to sit, talk listen, and communicate effectively. Be available 24 hours a day for community emergencies.
