Westminster-Canterbury on Chesapeake Bay is currently hiring a Connections Program Coordinator for Independent Living.

We have an immediate opening for:

What will I do?

Our Connections Program Coordinator implements and manages the therapeutic recreational needs of the residents participating in the Connections Program, to include physical, social, emotional, vocational, spiritual and cognitive needs, enabling them to function at their highest possible level. The Coordinator is responsible for producing a sustainable program that attracts participants from multiple levels of care and potential outside clients from the community.

  • Implements and manages the therapeutic recreational needs of the residents participating in the Connections Program.
  • Responsible for delivering daily therapeutic activities that meet the individual needs of participants as a part of their overall plan of care.
  • Producing a sustainable program that attracts participants from multiple levels of care, and potential outside clients from the community, and can be replicated in outside communities.
  • At the direction of the Enhanced Services Administrator or designee, assist with therapeutic recreation programming in Enhanced Services and The Chesapeake, Memory Care.
  • Develops and implements special projects to address the needs of residents.
  • Active in the planning and organizing of large and small group activities and one-on-one with individuals, making any necessary changes to assure successful results for all.
  • Provides documentation for programs & services. Provides pertinent input regarding changes and modifications in the activity plan of care that will assist in overall program and services improvements.
  • Documents participation or regression consistent with regulatory compliance in via electronic medical record.

What do I need for this role?

  • Healthcare related degree.
  • Experience in a health care or long term care setting and working with seniors.
  • One or more years of experience working with older individuals with dementia preferred.
  • Ability to utilize Microsoft Office and Google Apps software programs.
  • Ability to work well under pressure.
