Position: CORE Psych Rehab Specialist

Compensation: $42,000 per year (non-exempt position); plus, Sign-on Incentive

Location: Ronkonkoma, NY

Hours: Full time, 37.5 hours per week

Start Date: Immediately

Position Description:

The CORE Psych Rehab Specialist/Peer Support Specialist is a mobile, in the community practitioner, who has personal lived experience receiving mental health services. A willingness to share, to educate and to inspire others, and provide needed CORE Services. This will be achieved by establishing positive, strength-based, and supportive relationships with HARP enrolled (Medicaid Managed Care) individuals, whose chronic mental health and/or chronic substance use conditions are barriers to pursuing and sustaining meaningful life roles.

MHAW is committed to the education of all communities and our mental health workforce to promote trauma-informed practice, to promote prevention, as well as the best practices of early and effective intervention.

General Responsibilities:

• Promotion of recovery-oriented principles: of hope, self- efficacy, and community living skills through a shared personal experience of recovery.
• Co-creation of Individualized Service Plan based upon the goals and objectives identified through assessment.
• Individual coaching, Goal planning, and Support surrounding Independent and Social Living, Career Development, Peer and Family Support, and Managing Crisis and Stress.
• Assistance with daily living skills, support with social skills, and support with work related skills,
• Assistance in linking individual to community resources (employment or education), support with community integration and connectedness.
• Assistance with developing/increasing skills for coping, managing behavioral health symptoms and relapse prevention, using natural resources.
• Promoting and assisting with advocacy regarding entitlement benefits such as food, clothing, and shelter.

Qualifications:

The successful candidate should possess or be quickly able to acquire the following skills and abilities:

· A Bachelor’s Degree

· Two years of experience in working with individuals who have chronic behavioral health diagnoses (mental health and/or substance use)

· Strong engagement skills

· Must be comfortable working with individuals in their home and within the community, and providing telehealth services via laptop and smart phone

• Basic knowledge of the public health/behavioral health system including, but not limited to; mental health services and public entitlements
• Strong writing and computer skills, as an electronic record is used. Basic knowledge in computer applications such as Microsoft Word, Excel, Outlook and Social Media.
• Clean NY State Driver’s License as some driving with clients will be necessary
• Bi-lingual, English/Spanish is a plus; stipend given for assessed and verified bi-lingual proficiency
