We are looking for a Registered Dietician to join our PACE Program team in Tacoma, Washington.
Position Summary
The Program of All-Inclusive Care for the Elderly (PACE®) is a comprehensive, fully integrated, Provider 1 based health plan for the frailest and costliest members of our society – those who require a nursing home level of care. The PACE philosophy is centered on the belief that it is better for frail individuals and their families to be served in the community whenever possible. PACE programs are focused on delivering collaborative approaches to care and services that allow participants to live at home safely.
The Registered Dietitian assesses the nutritional status and dietary needs of PNW PACE program participants and plans for the provision of adequate dietary intake to enable client to achieve his/her optimal level of independent functioning. The Registered Dietitian also provides necessary consultation and teaching to participants, families and the health team as well as overseeing food services, including congregate and home delivered meals. All PNW PACE staff members must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elderly participants according to the assigned responsibilities and PNW PACE standards. Internal contacts include physicians, interdisciplinary team and other PNW PACE staff. External contacts include participants, their families or responsible parties, physicians, community health partners, and vendors.
