Spring Creek Nursing and Rehab is currently seeking a full-time Admissions Director for our long-term care (LTC) and skilled nursing facility. Scope of Work: Acts as liaison between Spring Creek Nursing and Rehabilitation and referral sources to coordinate an efficient and timely application and pre-admission process. Responsible for coordinating the admission process of inpatients/residents to Spring Creek. Assists in the coordination of pre-admission review program and admission process to ensure compliance with all federal, state, and local agencies for the Nursing facility.

Job Responsibilities include but not limited to the following:

  • Responsible for processing referrals and inquiries from hospitals, families and others.
  • Determines prior to admission the type of coverage to be used as the primary source of payment and verifies the benefits available for care.
  • Prepares admission packet for the case manager.
  • Enters pre-admission and admission information into the computer system/generates face sheet.
  • Maintains a close working relationship with the team to assure appropriate and efficient decisions about prospective admissions.
  • Responsible for timely review and communication of outcomes on all referrals.
  • Maintains constant communication with referral sources, internal departments and case managers.
  • Coordinates timely notification to the DON and Administrator of all transfers out of the facility to assure they are contacted and reassessed as needed.
  • Prepares weekly and monthly reports of all non-admit, lost referrals, and denial data and submits to CEO.
  • Makes follow up calls to all inquiries or to others to help determine the specific reason for denial, non-admit, lost referral, or transferred patient/resident's decision not to return to Spring Creek.
  • Gives tours to all interested parties and makes follow up calls the day after the tour to those that received the tour.
  • Plans facility events relating to any marketing efforts put forth by the facility.
  • Maintains confidentiality of patient and facility information at all times. Works closely with patients/families, employees of the business office, case management staff, nurse liaisons, marketing department, medical records and other patient related departments.
