Position: Registered Nurse - Director of Clinical Home Care

Responsibilities:

• Plan, organize, coordinate, evaluate, and administer all policies and procedures involved in the delivery of Home Care services in accordance with CMS Conditions of Participation for Home Health Agencies, RI Department of Health Rules and Regulations for Licensing Home Nursing Care Providers and Home Care Providers, and The Joint Commission or other accrediting agency.
• Serve as the Home Health Administrator and oversee the overall management and operations of the Home Care program.
• Provide leadership and supervision to home care staff, including nurses, aides, and other team members.
• Ensure compliance with regulatory requirements, quality standards, and best practices in home care.
• Develop and implement care plans based on comprehensive assessments of patients' needs and collaborate with interdisciplinary team members for coordinated care delivery.
• Monitor and evaluate the quality and effectiveness of home care services, identify areas for improvement, and implement necessary changes.
• Foster positive relationships with patients, families, caregivers, and community partners to promote patient satisfaction and positive outcomes.
• Provide education and support to patients and families on home care services, disease management, and self-care.
• Participate in performance improvement initiatives, staff development, and continuing education activities.
• Maintain accurate and complete documentation of home care services provided, including assessments, interventions, and outcomes.

Qualifications:

• Valid Registered Nurse (RN) license in the state of Rhode Island.
• Minimum of 1- 2 years of experience within healthcare administration or management.
• Experience in home care management preferred.
• BLS certification required.
• Ability to work day shifts (5x8-hour shifts from 08:00 to 16:30) and adapt to varying administrative responsibilities.
• Strong leadership, organizational, and communication skills.
• Knowledge of CMS Conditions of Participation for Home Health Agencies and regulatory requirements for home care providers.
• Ability to work independently and collaboratively in a fast-paced, dynamic healthcare environment.
