EISEP Case Manager Full Time
Organized, empathic, skilled communicator sought to work in interdisciplinary social services team environment as case manager to homebound older adults. Experience and Bachelor’s degree in Social Work or related field preferred. Reliable transportation required to conduct in-home visits with clients. Familiarity with local resources available to seniors and individuals with disabilities a plus. Responsible for assessing, planning, implementing, documenting, and monitoring supportive client care plans.
