Job description

Seeking an EXPERIENCEDE DDD Support Coordinator to work with individuals with developmental disabilities. Most hours can be remote with potential for full time in the future and face to face meetings once a month. A Support Coordinator's role is divided into 4 general functions: Individual Discovery, Plan Development, Coordination of Services and Monitoring. The Support Coordinator is responsible for the coordination and management of the overall services, including: development of Individual Support Plans (ISPs); coordination of interdisciplinary teams; coordinate service provision with assistance of the coordinator; allocation of monies for support services; and responsibility for maintaining high quality services.

Qualifications:

*MUST have experience working with clients with Developmental and Intellectual Disabilities

*BA Degree with 1 year of documented experience working with adults with developmental disabilities

*Clear a State, Federal, CARI and Central Registry Check

*Successfully complete any and all online trainings required prior to start of employment

*Meet all deadlines set by the Support Coordinator Supervisor regarding home visits and reports are documented with the Support Coordinator Monitoring Tool

*Ensure that All ISP'S and renewals are approved at least 30 days from date individual was enrolled

Job Type: Part-time

Pay: $30,000.00 - $60,000.00 per year
