Overview:
The purpose of mental health care coordination services is to engage priority individuals (recipients) and their natural supports who are not yet effectively connected with services to ensure service linkage, to enhance the delivery of treatment services and recovery supports and to improve outcomes among priority populations. Priority populations include: Children with SMI, SUD, or co-occurring disorders who demonstrate high utilization of acute care services, including CSU, inpatient and detox services. High utilization is defined as adults with 3 or more acute care admissions within 180 days; or, Adults with acute care admissions that last 16 days or longer. The Care Coordinator will be responsible for coordination of services, supports ,and cross system collaboration to ensure the individual’s needs are met.
Rate of Pay:$16.50 per hour (1-2 years of experience)
$17.50 per hour (3-5 years of experience)
$18.50 per hour ( 6+ years of experience)
Shift: Monday - Friday, 8:00 a.m. to 5:00 p.m.
