Health Care Specialist
About the Position:
We seek and experienced Health Care Specialist to work full-time in our residential group homes. The health care specialist will provide medical oversight to our Ossining residence. In this role, the HCS will participate in the assessment, planning and implementation of medical care, as well as providing oversight and training of direct care staff in monitoring the medical system. Additionally, the health care specialist will participate in team meetings and provide medically related in-service to program staff.
Locations: New Rochelle, NY.
Schedule: Monday-Friday 7:00 am -3:00 pm ( or schedule determined by program leadership)
Key responsibilities: (In addition to the responsibilities of the DSP)
- To provide leadership during their shift, setting the tone and ensuring that the standards of care are followed by all staff.
- To oversee all consumer medical issues, under the supervision of the LPN/RN. This includes being knowledgeable of all the consumer medical needs and ensure that all the staff have adequate knowledge of the medical needs in order to protect consumer health and safety.
- To be responsible for the ongoing oversight of the maintenance of the medical systems, ensuring that all anecdotals and other paperwork, including DA is properly filed and kept up to date.
- To ensure that there is proper scheduling and follow-up for all consumers’ medical appointments, in accordance with regulation.
- To be the liaison between the residence and all the medical providers and services, as well as the families.
- To oversee the medication inventory system as well as the medication cabinet checks.
- To know what level of supervision each consumer requires while sleeping, eating, fall precautions and performance of ADL’s and ensure that this level of supervision is provided to ensure the health and safety of the consumer.
- To promote the integration of health services, by ensuring that diets are followed, reinforces are appropriate, and that all staff are monitoring the medical issues and following up on MD recommendations.
- To help affiliate YAI’s AMAP course when assigned.
- To run AM medical meetings and participate in team meetings.
Education and Experience:
- Experience working within I/DD Field
