VA Maine Healthcare System's Togus VA Medical Center (VAMC) is a Joint Commission accredited, complexity Level 2 facility in Augusta, Maine that serves over 42,500 Veterans. Togus VAMC is a general medicine and teaching hospital that provides a full range of health services for Veterans, with state-of-the-art technology as well as education and research. Togus VAMC is a 67-operating bed facility with general medical, surgical, intermediate and mental health beds, as well as a 100 beds including hospice, dementia, long-stay, and skilled. Togus VAMC is the oldest Veterans' facility in the country, having opened in the fall of 1866. Togus campus encompasses 500+ acres of buildings and natural woodlands serving as natural habitats for Maine's wildlife. Additionally, the National Cemetery is located on the Togus campus. VA Maine Quality Management department is comprised of a highly skilled and diverse professionals responsible for Risk Management, Accreditation Readiness, Patient Safety, High Reliability Organization Program, System Redesign Team, Controlled Substance Coordination and oversight of quality metrics, including population health metrics like HEDIS and ORYX and VA specific measures such as SAIL. Member Duties : The Public Health AmeriCorps member would be designated as the project lead for VA Maine performance measure improvement effort. VA Maine is currently performing below the VA National average on preventative health measures (Prv90_3) with a goal of performance in the top 10% of our VISN. Current performance is at approx. 77 % with a target of 85%. The member will responsible for the following tasks: • Data review of current performance metrics and reporting to facility staff and leaders • Identify gaps in practice and/or process • Engage interdisciplinary teams in development of processes to implement best practices • Education regarding best practice, observations and feedback to frontline staff • Develop education for veterans using a variety of available platforms including social media, secure messaging, health fair • Evaluation of efforts with report out to executive leadership
