The Arc Lexington is currently seeking a Habilitation Coordinator to join our Quality Assurance team. The Habilitation Coordinator plays a key role in working with the people we support and the staff to develop and write individualized Residential and Day Habilitation Staff Action Plans based on the valued outcomes established by the person we support and their advocate.
Qualifications
- Bachelor's Degree in Human Services or related field plus at least one year working experience with people with developmental disabilities or an Associates Degree in Human Services or related field plus four years working experience with people with developmental disabilities
- Familiarity with Residential and/or Day Program operations is preferred
- Valid NYS Driver's Licenses is required
- Must have reliable transportation
Responsibilities
- Monitor Staff Action Plan implementation and the person we supports progress and satisfaction by visiting people at their homes or Day program
- Train staff and Managers as needed to ensure consistent, quality implementation of the Staff Action Plan as written
- Maintain awareness of community resources and available activities and ensure that people we support are being offered opportunities for community inclusion particular to his/her interests
- Develop and update Personal Expenditure Plans and Money Management Assessments in conjunction with the person we supports team
