The Health Home Care Coordinator (CC) provides services to patients in the Health Homes Program, including assessment of health goals and barriers, care planning, health promotion, community referrals, comprehensive transitional care, and individual and family support. The CC engages in face-to-face, telephonic, and in-home visits and primary and specialty care coordination. Successful applicants will have strong organizational skills, excellent interpersonal skills, and the ability to manage a caseload of 50-60 patients.



Education - Bachelor’s Degree in a social service or health care field such as social work, behavioral sciences, psychology, chemical dependency counseling, nursing, dementia practitioner, or related field. Master’s Degree is an added advantage.
Experience - At least two (2) years of experience working with individuals with a chronic condition or disability, or working in an outpatient primary care setting. One (1) or more years (s)’ experience in a multicultural health care clinic and direct service experience with bilingual/bicultural patients are preferred. Asian Pacific Islander language proficiency preferred.
Other Requirement(s) - Willingness to travel within the Seattle area for visits with patients in the community.
