JOB TITLE: Health Information Technician

SUMMARY: Responsible for assembling, organizing, and maintaining patient health information. Reviews information for scheduled patients, preparing the electronic medical record (EMR) for the patient’s encounter. Scans and indexes information into the EMR. Abstracts historical patient information from official sources and updates the patient’s EMR. Releases and obtains protected health information in accordance with the law and TBCHS policies.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following; other duties may be assigned:

Release of Protected Health Information - 30%

  • Handles requests for protected health information; dispersing information in accordance with law and TBCHS policies and procedures.
  • Maintains knowledge of departmental functions in accordance with federal and state law, as well as, TBCHS policies.

Assembling, Organizing, Maintaining Patient Health Information - 40%

  • Scans and indexes documents into patient’s EMR following TBCHS policies and procedures.
  • Abstracts and inputs protected health information from official records into the patient’s EMR.
  • Electronically records patient data for collection, storage, analysis, retrieval, and reporting.

Pre-Visit Preparation - 25%

  • Prepares patient’s EMR prior to scheduled office visits.
  • Completes pre-visit preparation.
  • Reminds patients of appointments.
  • Prepares forms for patient appointments.
  • Reconciles patient reported information into EMR.

Reporting - 5%

  • Reports the status of information requests, indexing, and information abstraction to the Health Information Manager.

Referral Tracking - 25%

  • Ensure that referral requests are processed in a timely manner.
  • Maintains ongoing tracking and appropriated documentation on referrals to promote team awareness and ensure patient safety.
  • Communicating to referral specialist for obtaining information from consult visit—this is done through the tracking letter.
  • Referral orders will be tracked daily for all STAT or urgent reports.
  • Referral tracking report will be processed monthly by each referral coordinator; looking at all referral order status’s according to specialty.
  • Referral order inquiry will be processed weekly for all routine referral orders; looking for consult notes received from specialist.

EDUCATION and/or EXPERIENCE: Proficient in Microsoft Office Suite, Internet/Intranet software with knowledge of Practice Management and Electronic Health Record systems. Two years Health Information experience required.

PHYSICAL DEMANDS: Light

TRAVEL: Travel from site to site may be required

Job Type: Full-time

Pay: $15.00 - $16.00 per hour
