Position Title: Health Navigator

Status: FULL-TIME (40 HOURS PER WEEK) FLSA EXEMPT
POSITION SUMMARY
Primary Accountability
The Health Navigator (HN) works in collaboration with the primary care provider and all members of the health care team, assessing healthcare needs and identifies community resources for patients. The HN serves as patient advocate and assists in identification and improvement of service delivery, work to decrease the spread of COVID-19 through specialized and individualistic work with clients. The Patient Navigator provides field-based case management services to clients. This person is the main point of contact for clients that builds strong relationships with CHP assigned clients to help clients stay engaged in COVID-19 services, medical care and adhere to their medications. The Health Navigators is committed to removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through health care and much needed social services and systems, promoting client health. They work closely with the Care Team, which may include doctors, nurses, Community Health Workers (CHWs), other community agencies and other clinical staff to support positive client health outcomes. HNs will work with clients who do not often fit well within the standard medical model. HNs will complete an intake interview for their clients, provide regular follow-up care and documentation and assist clients with referrals to other services, as necessary. This position will work to decrease the disparities in health due to COVID-19 and will help advocate for social and environmental needs as they arise. The HN will work closely with PCPs, Local Health Departments, and Community Based Organizations to provide wrap-around services to encourage clients to remain in their homes while positive with COVID-19. The HN will create a foundation of trust for continued care of socially vulnerable populations.

Duties/Responsibilities:
- Build rapport and connection with clients in an empathetic manner.
- Assist clients in referrals, scheduling, accessing, and utilizing telehealth services with their primary care provider. If the client does not have a PCP, work to get the client established with a PCP.
- Assist clients with determination of eligibility, registration, and sliding fee scale discounts for self and families for CHP referred services.
- Completes and submits insurance application, Charity Care, Patient Assistance Programs, etc., as needed for eligible clients and provide support to “self-pay” patients to help them pay their bills (both CHP bills and external bills, i.e., hospital, etc.)
- Screen clients for Social Determinant of Health Issues (SDOH) and make service referrals as appropriate.
- Provide clients with resource information by utilizing local database systems and the IPHA Resource List and others.
- Practice social distancing, mask wearing, and always use proper PPE.
- Able to identify symptoms of distress and act/intervene appropriately.
- Collects data for the program by being accurate and timely in documentation of client encounters.
- Protect HIPAA of clients participating in the HN and all other related programs.
- Participate in all IPHA and other assigned CHP required meetings and trainings related to the HN program.
- Establishes close relationships with and serves as primary point of contact for clients
- Visits clients’ homes on a regular basis as needed or in collaboration with CHW efforts
- Deliver in-home weekly or monthly health education and promote health program self-management to clients
- Communicate with Care Team members (CHWs, Care Coordinators, primary care physicians and other health care providers) to facilitate client care
- Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements
- Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance
- Maintain strict confidentiality in accordance with agency policies
- Meet with clients after primary care physician appointments to review and update care plan with the Care Coordinator, schedule future appointment, referral, and education patients on keeping their appointments.
- Other duties as assigned.
COMPENSATION:

FLSA STATUS
This position is considered exempt under the Fair Labor Standards Act and is therefore not eligible for overtime pay. This position is recognized as a member of the Executive Management Team for the development division and exercises independent judgment on a variety of issues impacting CHP. This position will be required to travel to the different CHP locations on occasion and is eligible for mileage reimbursement according to the rules of the IRS.

Organizational Duties:
Meet with Care Team (including, but not limited to, CHWs, Care Coordinator and primary care provider) to discuss client care issues and needs and facilitate client health care· Maintain documentation of all client encounters and complete reporting requirements according to organization standards · Track client information, schedules, files, and forms in a confidential manner· Track client attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary· Attend and represent the organization at local meeting, in-service trainings, and meetings at the request of or with the approval of supervisor
Education and Experience:
- Minimum high school degree or some college education
- Strong understanding of cultural competency with the target population
- Bilingual (English/Spanish) preferred
- Computer literacy desirable
- Must have experience working in community work. This can include advocacy, volunteering, etc.
- Must be willing to see the inherent worth and dignity of each client.

PHYSICAL DEMANDS:
Most of the work the HN will do, will be in at a CHP Health Center Site or in their personal home-based setting during remote work due to COVID-19 where applicable. From time to time HN will meet face to face with clients. The HN will meet virtually with most clients. This virtual meeting may include phone calls, emails, texting, or video conferencing. There will be times that clients need items delivered to their homes such as food. This would include lifting and carrying of 20 pounds. The HN may deal with time sensitive requests. The HN may be exposed to outdoor temperature extremes, including dust. The HN must follow Covid-19 personal protection guidelines.• Physical demands associated with office work• Some local travel• Some evening and weekend work required• This position requires the manual dexterity sufficient to operate phones, computers, and other office equipment, and the ability to sit and to use a computer for significant periods of time. • This person must speak clearly using the English language to accurately convey information. • Work performed in an office setting as applicable determined by activities. • Work requires above average lifting, standing, long periods of sitting or other office discomfort and occasional weather exposure in outreach settings and local driving. • Must be able to lift to 50 lbs. Manual dexterity is required.• Must possess normal vision or correctible to within normal ranges.• Work may include extended periods of time viewing a computer monitor and or operating a keyboard.• OSHA Category I Position Classification for occupational exposure to blood and other possibly • infectious materials. • Some direct physical contact with patients and airborne contaminants.• Some work in the community, in groups, or in receptive in-home environments.
