Position Description
Home Care Hospice Aide:

Ogeechee Area Hospice is looking for qualified and licensed CNAs to work with our Home Care patients. Although prior experience in a hospice type environment is preferred, it is not required. This position requires that the employee be self motivated and can work independently. The position will allow the employee to travel in a certain area within Ogeechee Area Hospice's servicing area providing care and assistance to our home bound patients. While the employee will be using their own vehicle for home visits, Ogeechee Are Hospice does reimburse for mileage. Motor Vehicle Report, Background Check, and Drug Screen will be conducted at the time of hire.
General Role Description:
An individual who, under the direction of a Registered Nurse, provides personal care services within the guidelines of the Interdisciplinary Team Plan of Care to patients in Home Care. The Hospice Aid reports to a manager in Home Care. The Hospice Aide demonstrates commitment to the mission and values of Ogeechee Area Hospice.
Qualifications:
Education, experience, skills and required documentation:
1. Demonstrates competency in hospice care and required skills by passing competency and skills examination administered by a Registered Nurse
2. Demonstrates ability to read and write short correspondence
3. Demonstrates math skills in order to be able to keep accurate activity records including Intake and Output.
4. Demonstrates ability to carry out written and verbal instructions
5. Possesses current Georgia Driver’s License, proof of automobile insurance required by state, clear screenings required by agency, as described in the personnel policies of agency and reliable transportation.
6. Must have up to date certification from approved training program, and at least two years experience in Hospice, Home Health, Hospital and/or Long Term Care Facility preferred.
7. High school education or GED required
8. Demonstrates basic computer and math skills.
Physical Demands Necessary for Role Functions:
1. Physical exertion including walking, lifting, pulling, step climbing and flexible hours. Must be able to regularly stand, walk, reach, stoop, kneel and use medical equipment.
2. Requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.
3. Must have close vision, distance vision and depth perception with corrective lenses if needed.
4. The employee is regularly required to talk and hear.
5. Requires moderate amount of computer work throughout the work schedule.
6. Must be able to drive approximately 20 to 500 miles per week when assigned to Home Care.
Work Environment:
This position requires working Monday through Friday, 8:30am until 5:00pm. Occasional weekends and holidays will apply. Work exposure requires safety measures and standards precautions including are not limited to: communicable diseases, blood and other body fluids, needles, and other medical equipment, inclement weather and visiting or home animals. The use of a personal vehicle is required for patient visits.
