Job Description:

  • Conduct assessments and aide supervisions in patients' homes

  • Assess patient head to toe including medications & diagnoses and document

  • Supervise home health aide/personal care aide in all duties

  • Create a Plan of Care for aide to follow and orient to POC

Position Requirements:

  • Current and Valid NYS RN License

  • Must be able to travel to patient cases within the required area(s)

  • Must carry Medical Professional Liability coverage

  • Intermediate ability to use computerized systems such as Gmail, Microsoft Word, Excel, and to navigate the DOH UAS health care system

Preferred Qualifications:

  • Bilingual Spanish/English

  • Medicaid, Medicare, Managed Care experience

  • Experience in Home & Community based or Long Term Care services delivery

  • Experience in the use of UAS-NY Application

  • Experience working with the needs of vulnerable populations and passionate about patient care

  • Excellent written communication/documentation skills
