For this position, some home health/OASIS experience is required.

This position if for the central/north Phoenix, Paradise Valley and Scottsdale areas, bous if you will cover Cave Creek area.

Requirements:

Desire to be a part of a fantastic team!

Active PT License

BLS, TB Screening, Level 1 Fingerprint Clearance

Comfortable with electronic charting

Excellent assessment skills

PTA available to maximize your productivity

Responsibilities:

Performing assessments and developing plan of care.

Performing home visits that include one on one teaching and working with patients and families.

Some PT SOC will be required.

Job Type: Part-time

Pay: $75.00 - $95.00 per hour
