Position Summary

The Quality Educator is responsible for improving patient outcomes by delivering quality, education, and performance improvement activities. Responsible for development, implementation and evaluation of the education plans and programs. This role requires competency in clinical care delivery, clinical processes, OASIS accuracy, evidence-based practice, research and evaluation, business operations, current and emerging regulatory requirements, federal mandates, and national standards

Essential Functions

Creates and participates in the orientation and ongoing education, training, associate development for the nursing and therapy team

Manages preceptor support and development, to include coaching and mentoring.

Instructs clinicians to perform duties within the Home Health framework.

Conducts classroom and 1:1 associate education, training, and associate development.

Demonstrates critical thinking, and applies the principles of adult learning, teaching/learning theories and age appropriate teaching strategies to the implementation of educational programs.

Partners with Clinical Leadership to assist with annual clinical competency evaluation of clinicians and contract therapists. Plans joint visits with clinical associates one or more times per year, and as needed

Plans and provides educational programs for the clinical team; provides educational orientation and assists with other educational needs as required.

Performs medical record audits to ensure documentation compliance and adherence to Medicare, Medicaid, TJC, State and Federal regulations and organizational policies; Evaluates documentation to ensure regulatory compliance, clinical outcomes, and OASIS accuracy; Provides education on clinical documentation and best practice.

Monitors CMS Home Health Quality Reporting Program (HHQRP) clinical outcomes to identify performance improvement opportunities. Partners with Clinical Team to develop and implement performance improvement processes.

Monitors ongoing clinical outcome data to achieve quality outcomes.

Participates in and/or leads Performance Improvement workgroups.

Ensures effective learning experiences, competency assessment, and validation.

Develops programs and educational content that reflects practice changes, industry trends, best practices, regulatory requirements and agency competency validation requirements.

Job Requirements:

VA RN license in good standing

BSN preferred

Minimum of 5 years of Home Care experience

COS – C (OASIS). preferred , must obtain first year.

Excellent clinical, oral and written communication, interpersonal, OASIS accuracy, and customer services skills

Previous leadership and/or management experience preferred.
