Position Summary:
The Care Coordinator will serve as a liaison to staff members, physicians, and health care facilities and act as an ongoing resource for the Company.

Position Duties:
- Assist with the maintenance of accurate records on all referral sources, status of existing or pending referrals, admissions, discharges, and hospitalized patients as needed
- Ensure a primary liaison is maintained between the Company and referral sources
- Compile patient information from referral sources including specific patient demographics, insurance and/or payor sources to ensure quality of communications, information and care for patients
- Assist with ensuring patients meet the face-to-face encounter regulations
- Provide skilled nursing care to patients in accordance with physician's orders and in compliance with company policies/procedures, state and federal regulations if requested
- Request additional information via telephone for referrals that are incomplete
- Complete clinical record audits
- Manage and document phone calls from physicians, clinicians, patients, referral sources and communicate patient updates to clinicians
- Other duties as assigned
Other Requirements:
- Effective written and verbal communication skills
- Must be able to speak and write in English
- Unrestricted driver’s license
- Reliable transportation
- Current automobile insurance according to Company policy
Licensing/Certification/Education/Experience Requirements:
- 3 years’ experience as a licensed practical nurse or RN preferred
- 1 year home health experience required
- 1 year case management preferred
- Current practical nursing license
- Registered nurse license preferred
- CPR certification
Physical Requirements:
- Ability to lift to 15 pounds
