Position Summary:

The Care Coordinator will serve as a liaison to staff members, physicians, and health care facilities and act as an ongoing resource for the Company.

Position Duties:

• Assist with the maintenance of accurate records on all referral sources, status of existing or pending referrals, admissions, discharges, and hospitalized patients as needed
• Ensure a primary liaison is maintained between the Company and referral sources
• Compile patient information from referral sources including specific patient demographics, insurance and/or payor sources to ensure quality of communications, information and care for patients
• Assist with ensuring patients meet the face-to-face encounter regulations
• Provide skilled nursing care to patients in accordance with physician's orders and in compliance with company policies/procedures, state and federal regulations if requested
• Request additional information via telephone for referrals that are incomplete
• Complete clinical record audits
• Manage and document phone calls from physicians, clinicians, patients, referral sources and communicate patient updates to clinicians
• Other duties as assigned

Other Requirements:

• Effective written and verbal communication skills
• Must be able to speak and write in English
• Unrestricted driver’s license
• Reliable transportation
• Current automobile insurance according to Company policy

Licensing/Certification/Education/Experience Requirements:

• 3 years’ experience as a licensed practical nurse or RN preferred
• 1 year home health experience required
• 1 year case management preferred
• Current practical nursing license
• Registered nurse license preferred
• CPR certification

Physical Requirements:

• Ability to lift to 15 pounds
