Job Summary
The Children’s Hospital of Philadelphia’s (CHOP) Community Asthma Prevention Program (CAPP) was launched in 1997 by Founder and Medical Director Dr. Tyra Bryant-Stephens specifically to support these children. CAPP operates a Home Visitor Program, now expanded to all of Philadelphia and parts of Delaware and Montgomery counties, utilizing peer community health workers to teach in-home classes and provide families with supplies to mitigate asthma triggers in the home environment, with the goal of improving asthma-related outcomes in children.
This Home Visitor role will be working specifically with families in Chester, PA. Their primary responsibility is to work with families of children with persistent asthma, providing services as identified by the health care team. The Home Visitor will follow families until goals reached as defined by the health care team. This includes but is not limited to: asthma education in the home, home environmental mitigation, and care coordination. The Home Visitor is responsible for a minimum caseload and caseload follow-up.
Job Responsibilities
- Make at least 10 home visits each week and/or enrolls 3 children each week
- Responsible for maintaining Microsoft Outlook calendar which documents daily schedule for site manager and CAPP Medical director
- Follows all HIPPA and confidentiality regulations regarding protection of patients.
- Attend all regularly scheduled staff meetings, including CAPP team meeting and the Home Visitor meeting
- Keeps record and collects data as requested by managers
- Maintains positive relationship with caregivers as determined by patient satisfaction
Travel to community sites for health fairs and events when appropriate
Job Responsibilities (Continued)
- Conduct initial assessment of home to evaluate for asthma triggers, conduct asthma education classes in the home, demonstrate and supervise environmental interventions in the home. Assist patients and families in identifying education, information, agency referrals and advocacy to assist in managing medical and social needs. Maintain caseload of 50 families. Call to set up appointments with eligible community members for the home visitor program. Follow all protocols
- Connect families to community agencies, social service organizations, insurance providers and other resources according to identified needs through in-person or virtual assistance.
- Conduct regular follow-up in-person or virtual visits to reinforce education and assess maintenance of intervention.
- Document all visits, activities with family, supplies used, etc and enter data into database and electronic health record. Responsible for appropriate use of incentives and intervention supplies, maintaining organization of storage closet and documentation of use of supplies and incentives
- Assist in training new Home Visitor staff.
- Attend all CAPP staff meetings.
Required Education and Experience
Required Education:
- High School diploma or GED
Required Experience:
- At least three (3) years working in community-based programs, community health

- Personal experience with medical complexity preferred but not required
