Job description
As a Destiny Healthcare Services Field Supervisor you are responsible for assessing all clients before services start. The Field Supervisor is also responsible for home visits.
•You will observe the caregiver in the clients home to insure the caregiver is following the clients care plan, client’s schedule, and the agency rules.
•Creates a weekly schedule that assures the clients receive timely assessments and home visits for all 4 locations.
•Follow up on all home complaints.
•Completes the appropriate documentation following each visit and enters(uploads) to the software Kantime.
•Follow up with all in office supervisors/managers to receive any new tasks.
•Report to Agency Administrator
Overall provide non-clinical guidance and training to home care aide staff in the field and will evaluate their performance in the field
Field Supervisors will need to have their own vehicle to travel to clients home daily. The Field Supervisor will be paid per hour and also receive mileage reimbursement per day.
