JOB SUMMARY

  • Evaluate patients conditions for swallowing or cognitive deficits.
  • Help patients in their goals to improve Cognition or Swallowing.
  • Provides professional physical therapy diagnostic and therapeutic services by collaborating with physician and interdisciplinary team members to develop and manage the patient's plan of care.
  • Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression.
  • Provides for patient education to include the development of comprehensive and progressive home programs with instruction to the patient, family and other caregivers
  • Develops comprehensive goals for patient treatment to maximize patient outcomes.
  • Instructs and advises clients, family and other health team personnel including, when appropriate, home health aides, in the phases of physical therapy in which they may assist the client.
  • Provides documentation according to policy.
  • Ensure compliance with relevant laws, regulations, or standards.
  • Other duties as assigned.
EDUCATION

  • Master's degree in Speech Therapy or a foreign degree equivalent as accepted by US Citizenship and Immigration Services or a Bachelor's Degree or foreign degree equivalent plus 2 years of progressive, post-baccalaureate experience.
  • Graduate of an accredited Speech Language Pathologist program.
EXPERIENCE

  • Minimum of one year of experience post-license in speech therapy.
  • Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population, and to a lesser degree the pediatric and other adult population.
  • Demonstrated knowledge of the principles of growth and development over the life span.
  • Able to assess data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
CERTIFICATIONS/LICENSURE

  • Valid New York State license as a Speech Language Pathologist.
KNOWLEDGE AND SKILLS
  • Excellent communication skills and the ability to interact well with diverse individuals.
  • Experience with territory management, strong presentation skills, performance management, building relationships, emphasizing excellence, negotiation, results driven, sales planning and execution.
  • Self-starter who requires minimal supervision
