Access North is looking for a Part-Time Independent Living Coordinator in the Brainerd Area. The primary service area for this position includes Crow Wing, Cass and Aitkin Counties.

The Independent Living Coordinator is a professional position coordinating consumer services, providing independent living skills training, providing advocacy services, and providing information and referral.

Responsibilities of the role are:

· Provide Independent Living Skills Training to adults and youth with disabilities within the Access North office, their home and community.

· Provide the core services: Information & Referral, Advocacy, Peer Support, Independent Living Skills Training, and Transition services to consumers living throughout the service area.

· Maintain an up-to-date expertise in disability rights legislation, regulations, and policies relevant to people with disabilities and advocate as appropriate.

· Facilitate educational and support groups for consumers and community members.

· Supervise, plan, train, teach and assist each consumer in learning activities associated with their personal goals. Writing and implementing the consumer's Independent Living Plan focusing on individual choice.

· Maintain consumer records such as: Case/progress notes, Documentation of consumer performance/progress § Disposition of incident reports, Enter consumer data electronically, Keep up to date Consumer Service Records in compliance with federal guidelines

Education and Experience

Required: A minimum of three year’s experience working with disability issues and experience working with people with diverse disabilities.

Preferred: A Bachelor's degree in a human service field, and some experience working with disability issues is preferred
