We are looking for an experienced Home Health Intake Coordinator responsible for greeting incoming patients, recording their personal details, gathering medical histories, and obtaining any health insurance information they have.

Position Description:
Take new referrals as per referral process. Coordinate client care following guidelines established by the RN Clinical Manager, payer authorization, physician orders, and home care regulations. Run daily reports and ensure client coverage. Document and communicate pertinent information to client, caregivers, payers and field staff. Communicate with clients/families regarding desired services.

Education and Experience:

  • High-school diploma or equivalent

· 2 years’ experience as an Intake Coordinator

  • 1-year Advanced medical terminology knowledge
  • 2-years Home Health Experience
  • Excellent attention to detail
  • Excellent customer service skills
  • Strong computer skills
  • Pass background check
  • Ability to interact effectively with all levels of personnel
  • Effective organizational skills

Salary: From $25.00/hr.

Schedule:

  • 8-hour shift Monday to Friday in office (not remote) Occasional Saturday rotation

Job Type: Full-time

Pay: Up to $25.00 per hour
