Case Manager - LEAD
The Law Enforcement Assisted Diversion Program (LEAD) is a National Model in which law enforcement officers exercise discretionary authority at point of contact with a community member to divert individuals to a community based, harm reduction intervention program for law violations driven by unmet behavioral health needs. These community members are referred to a trauma-informed, intensive case management program that provides a wide range of support services and linkages to treatment. The program utilizes a harm reduction approach.
Position Title: Case Manager
Position Type: Full Time; 40 Hours Weekly (M-F; Some evenings/weekends required)

Requirements:
- Bachelor’s Degree in Social Work or related field
- Experience working with Substance Abuse
- Proficient with computer programs including Excel, Power Point, Word
- Experience and understanding of Law Enforcement
Responsibilities:
- Report to assigned Ocean County Police Departments throughout the week
- Accept referrals from participating municipal law enforcement agencies and triage needs
- Provide needs assessment, linkage and brief case management to referrals
- Work as part of team
- Conduct community outreaches independently and as part of team
- Maintain information/data spreadsheets
- Coordinate and consult with law enforcement officers and law enforcement command staff
- Complete documentation as required and communicate with other program staff
- Participate in law enforcement meetings and education
- Work with student interns to enhance their educational experience
