We're Hiring for a Hospital Liaison - RN
The Hospital Liaison - RN will work in and around the Stamford Hospital area and will manage all clinical aspects of assigned accounts for our continuum of care. This person will manage the clinical care coordination and work with the Clinical Directors and Intake as needed on the continued development of post-acute care facilities in the service area. The Hospital Liaison is directly involved in managing the multiple clinical elements that comprise a person’s successful transition from one level of care to another., Assess clinical status, and eligibility for the identified benefit, discusses goals of care, and makes recommendations of appropriate service selection. This candidate should demonstrate exemplary communication, organizational skills, and knowledge of clinical eligibility with clinical assessment skills to identify the proper disposition of care needs.

Position Requirements:
- A Licensed Registered Nurse in the state of Connecticut is required.
- Minimum of 1-3 years of clinical experience with experience in Hospice, Home Care and or Palliative Care.
- Knowledge of and ability to promote and educate accounts, families, and patients on all aspects of home care, palliative & hospice, and private duty services.
- Ability to assess patient clinical status and discuss care options for the state of illness.
- Ability to facilitate discussion of care goals with patients and families.
- Expert clinical knowledge of hospice regulations and the hospice philosophy of death and dying.
- Comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources based upon clinical assessment of patient condition.
- Knowledge of payers and eligibility requirements based on clinical status.
Responsibilities:
- Assesses clinical status, and eligibility for the identified benefit, to determine the appropriateness for Hospice admission based on Hospice Criteria.,
- Discusses clinical goals of care and makes recommendations for appropriate service selection.
- Performs full clinical assessment and handles GIP admissions and visits.
- Clinical focus on ensuring the patient's eligibility, required consent forms, correct coding and billing documentation.
- Develops and manages relationships with referral partners within the assigned post-acute accounts and in the community.
- Identification and prioritization of potential referral partners, including decision-makers within each account.
- Commutable to all geographic territories as needed for assessments, training, orientation and referral source visits
- Occasional weekend coverage & holidays (as needed)
