Schedule: Tuesday to Saturday from 8 am - 4:30 pm

The first face of Saint Francis: responding to referrals to hospice in various locations, homes, nursing homes and hospitals to present St Francis services and Hospice Philosophy to their patients and their loved ones. Gather information and records for review by hospice physicians to determine hospice appropriateness. Develop and maintain excellent professional relationships with referral sources and facility staff. Educating and explaining hospice services so that patient and family may have informed consent and assist in completion of admissions paperwork. Prepare patient and family for transition and next steps as they enter into the Hospice journey.

EDUCATION AND/OR TRAINING REQUIRED: Bachelor’s degree preferred but not required. Other degrees or vocational certification considered, MSW a bonus, bilingual a plus. Previous healthcare sales or service experience preferred. Previous Emergency Medical Services (EMT, Paramedic) Medical Social Work hospital, physician practice, LTC or ALF experience preferred

CERTIFICATIONS/LICENSES REQUIRED: Commensurate with education.

EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED: Must be an empathetic person and excellent communicator in verbal and written communication, with strong interpersonal skills. Must be able to use EMR and other computer programs with ease. Maintain clinical and professional competence with emphasis on expertise in clinical appropriateness, eligibility requirements and coordination of care. Must be proactive in reaching out to patient’s referral sources and staff. Must maintain a reliable means of transportation

PRIMARY RESPONSIBILITIES:

6. Serves as a liaison between Hospice of St. Francis and referral sources, including but not limited to, hospitals, case managers, emergency department and other identified hospital staff.
