The Licensed Mental Health Professional (Clinical Treatment Coordinator/CTC) works on the inpatient mental health units at MHealth Fairvew – Riverside campus and partners with the physician and interdisciplinary team to navigate the patient/family through the inpatient treatment episode. This role conducts mental health assessments, completes treatment planning, provides evidenced based therapeutic interventions, and conducts discharge planning and community re-entry.
A variety of part-time and full-time opportunities and shifts are available on both the pediatric or adult units. Individual and Group supervision opportunities available for those who have their LGSW or LPC!

Demonstrates clinical competence in the treatment planning process.
- Coordinates treatment planning and leads management of each case.
- Coordinates/facilitates the development of treatment plan by partnering with the physician and collaborates with patient, family and interdisciplinary team.
- Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
- Implements, reassesses and adjust treatment plan as necessary.
Demonstrates clinical competence to meet patient needs.
- Completes an interdisciplinary assessment with each patient.
- Provides individual, group, and/or family therapy.
- Provides clinically appropriate psychoeducation.
- Collaborates with patient’s treatment team.
- Documents thoroughly, timely, legible and clinically appropriate.
Demonstrates competence in developing a clinically relevant transition/discharge plan.
- Partners with physician and family to provide leadership to the inter-disciplinary team.
- Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
Demonstrates knowledge of utilization review and collaborates with the Utilization Management Specialists to insure the ongoing, comprehensive monitoring and evaluation of patient care, by concurrent and retrospective review to assure appropriate reimbursement.
Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery.
- Incorporates data and information into practice to continually improve care and practice so that patient outcomes are improved.
- Promotes a culture of evidence-based practice by participation in any of, but not limited to, the following activities:
- Utilizing evidence-based policies in clinical practice
- Attends and participates in learning opportunities about evidence-based practice
- Membership in hospital or unit based practice council/research council
- Evidence based practice project (e.g. policy development or poster presentation)
- Journal club, journal article critique and circulation
- Literature review
- Development of standard/pathway/guideline for patient care
- Presentation
- Research study
Responds and participates in Behavior Control Situations (BCS) and CPR.
Qualifications:
Required:
Education
- Master’s degree in psychology, counseling, social work, marriage and family therapy or a related field.
Experience
- Two years’ therapy, case management, or assessment experience, including providing face to face therapeutic/clinical services to adult, or child/adolescent clients.
License/Certification/Registration
- Basic Life Support Certification prior to end of orientation.
- A minimum of one of the below is required:
- Licensed Independent Social Worker (LISW)
- Licensed Independent Clinical Social Worker (LICSW)
- Licensed Graduate Social Worker (LGSW)
- Licensed Psychologist (LP)
- Licensed Professional Counselor (LPC)
- LPCC Licensing Candidate
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Associate Marriage and Family Therapist (LAMFT) OR
- Licensed Marriage and Family Therapist (LMFT)
