Transitional Care Management provides comprehensive healthcare management services to affiliated and contracted nursing homes. We enhance patient satisfaction, reduce cost and improve outcomes to those centers we serve. We have a new client in Freeport, IL and we are looking for an experienced IL Licensed Nursing Home Administrator (LNHA) to be a part of the team.
About the center:
This is a 143 bed SNF with a great reputation in the community. Serving the intermediate care and skilled population, we are looking for an administrator that can be a change agent and lead this stable team. The center has:
- 25 bed dedicated memory care unit
- 5-star rating for our staffing ratios
- Our turnover rate is 20% lower than the national average
- a DON that has been a part of the team for 20+ years
About you:
The Nursing Home Administrator (LNHA) sets the pace for the skilled nursing facility. You lead by example and you know first impressions matter. People remember you, respect you and want to work with you. You make an impact in your patent’s lives and you strive to make a difference.
- Your number one goal? You will partner with us to revise clinical programming and marketing initiatives to focus on rebuilding census.
- This will involve interfacing with our union and representing the center at county board meetings.
What we will need from you:
- 3-5+ years of experience as an LNHA in a skilled nursing facility
- Ability to build a team and potentially bring some with you
- A love for what you do, and your education and credentialing reflects that
