Responsibilities:

The Licensed Practical Nurse (LPN) Ambulatory Care will work Full Time, Days and provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician's Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN, Advanced serves as a resource for other team members and is active in the orientation of new staff.

  • The Licensed Practical Nurse collects and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback.
  • The Licensed Practical Nurse evaluates health data to identify patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.
  • The Licensed Practical Nurse implements and documents specified interventions with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.
  • The Licensed Practical Nurse identifies practice problems and consistently seeks solutions through appropriate communication channels.
  • The Licensed Practical Nurse delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor.
  • The Licensed Practical Nurse demonstrates operational problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation.
Qualifications:
Bassett Healthcare Network follows the New York State and New York State Department of Health COVID-19 vaccine mandates for healthcare workers. This mandate requires COVID-19 vaccinations for personnel working for or on behalf of our hospitals and nursing homes. The New York State mandate allows for medical exemptions and religious accommodations, when appropriate. Education:

  • Graduation from an accredited Practical Nursing program is required.

Experience:

  • Minimum of 2 years experience in current area of ambulatory practice.
  • Minimum of one year with Bassett Healthcare.

Licensure/Certificates:

  • Current NYS Licensure as a Practical Nurse.
  • Additional credentials are determined by specific work unit.

Skills:

  • Ability to determine patient status and take appropriate action.
  • Proficiency is required nursing skills relevant to area of practice.
  • Ability to multi-task effectively in a rapidly changing environment.
  • Ability to lead approved work unit initiatives.
  • Services a resource to all staff and mentor to LPN Ambulatory Care.
  • Ability to lead approved work unit initiatives.

Physical Requirements:

  • Walking, standing, squatting and kneeling required throughout the shift.
  • Ability to lift and position patients required.
  • Category I risk exposure as defined by OSHA.
  • Must be able to identify and differentiate colors.
