LPN

$1,500 Hiring Bonus

$3,600 Retention Bonus ($300/mo)

Summary: Responsible for medical treatment, monitoring and medication administration of consumers. Positions also provides supervision, training, personal care and transportation.

Essential Duties and Responsibilities include the following. Other duties may be assigned to meet consumer and agency needs.

· Monitors general health condition of consumer, including vital signs, performing basic medical treatments dispensing medications.

· May order medication under the supervision of Prescriber and according to State Law.

· Maintains facility to ensure sanitary conditions.

· Comply with federal, state and company policies, procedures and regulations.

• Maintains confidentiality of all information and complies with HIPAA regulations
• Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.

Qualification Requirements:

· Must be able to communicate clearly and accurately with a variety of individuals

· Must work well in a team environment

· Work with staff, consumers and others in a professional, cooperative, ethical, respectful and effective manner.

· Ability to work with a diverse group of people

· Must have valid driver’s license and Automobile Insurance.

Education and/or Experience: Current State Licensure, prior computer/PC experience using Computerized Medical Records and scheduling systems is preferred, but not required.

Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Mobility and ability to sit for long periods of time, use a standard computer keyboard, bend, reach and lift up to 50 pounds are required in this position.

Job Type: Full-time

Pay: $21.00 - $24.00 per hour
