Our Countryside, Gen Center and Cornerstone Adult Day Health Centers in Fall River and Raynham, MA have an exciting Per Diem Float opportunity for a Licensed Practical Nurse (LPN). The hours are 8:00 a.m. - 4:00 p.m. Monday through Friday; with no weekend or no major holiday shifts.

Job Description:

• Assist Program RN as liaison between physician and participants for medical information and service authorization of participants.
• Complete medical assessment forms as designated by RN.
• Monitor participant’s health status.
• Provide case management services as designated by RN.
• Assist Program RN in reviewing and supervising participant’s personal care services.
• Attend monthly supervisory meetings with RN.
• Assist RN with other services involving health issues of participants.
• Assist RN in writing monthly nursing progress notes in each participant’s record.
• Assist the Program Coordinator with the implementation of policies and procedures.
• Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator.
• Respond to reasonable expectation of the Agency and/or supervisor.

Education and Requirements:

• Current licensure in the Commonwealth of Massachusetts Board of Registration in good standing,
• CPR certification.
• Two years of experience working with aging and disabled population required; previous experience working with mental health, dementia, veterans', dual diagnosis preferred.
